Senior Backend Software Engineer (LATAM Only)

**Contract:**  International contractor —  **$35/hour** Language: English (mandatory fluency, written \& spoken) **Reports to:**  CTO \+ Engineering Team **About the Role** We are seeking a highly seasoned Senior Backend Engineer with \+5 years of experience building and shipping production backends with PostgreSQL. You are an advanced backend engineer with strong database skills, experience integrating external APIs, a strong product focus, and you are comfortable thinking on your feet in a fast\-paced and dynamic startup environment. Experience integrating with medical devices and systems is highly desired. **Tech Stack** * PostgreSQL (modeling, security, RLS, performance, transactions, triggers) * TypeScript * Next.js / Node.js * External API integrations * AI/LLM programmatic integrations * Advanced testing \+ reliability * **Nice boosts:**  Supabase, DevOps, React, AWS, Python, HL7/FHIR, Temporal/Airflow, medical device integrations **Required** * Expert level proficiency with PostgreSQL Database (modeling, security/authentication/authorization/RLS, performance, transactions, triggers, scaling, data history, etc) * TypeScript * NextJS/NodeJS * Advanced experience integrating with external APIs. * AI Integration: Experience programmatically interacting with LLMs or other advanced AI models * Solid CS fundamentals * Advanced testing experience and maintaining high reliability **Strongly Preferred** * Experienced with Supabase (especially Row Level Security and Realtime features) * DevOps expertise, implementation experience, and uptime reliability * AI Training: Capability to help craft our long term AI strategy, along with training and customizing open source AI models based on medical data * Solid proficiency with React (we’re a NextJS shop; since we’re a small team you may be asked to contribute to the front\-end) **Bonus** * AWS * Experience with backend workflow tools such as temporal.io, airflow, etc. * Medical device integrations (Immunoassay, Ultrasound, etc) * Medical data (i.e. HL7, FHIR \& HIPAA compliance) * Python **How We Work** * Small, high\-impact team * Fast decision\-making * High autonomy and ownership * Clear, async\-friendly communication * Focus on real outcomes, not bureaucracy
