Lead Backend Engineer (Python) — Silicon Valley Healthcare AI Company (Remote, Latin America)

Areti Health is a fast\-growing Silicon Valley healthcare technology company. We have built the only full\-service clinical trial patient recruitment AI agent. Backed by leading venture capital firms with a $6\.6M funding round just closed, we are on a mission to transform clinical research through technology that allows pharmaceutical, biotech, and medical device companies to bring their therapies to patients faster and save lives. We achieve industry\-leading conversion rates: up to 10x improvement over human\-driven processes with up to 8x operational cost reduction. Coupled with our seamless medical record access and mass outreach capabilities, we compress recruitment timelines from many months to a few weeks across multiple therapeutic areas. Our AI\-first system matches prospective patients, answers their questions based on clinical trial documentation using voice and chat AI, schedules appointments, collects patient information, runs pre\-qualification questionnaires, assists with patient onboarding, nurtures scheduled patients, and integrates with research site systems – all fully autonomously. We work with some of the largest US clinical research players, including the \#1 commercial clinical research organization ($16B in revenue), the \#1 academic clinical research organization (trials at 1,100 sites), and the \#1 clinical research site network. You will join a team of 20, working closely with the founder who brings a prior $4B healthcare technology exit, Y Combinator founder experience, a Stanford PhD, and three Silicon Valley CTO stints. We offer competitive salary, stock options, and paid time off. **What you will do** * Scale the product to support thousands of clinical trials * Work directly on AI applications daily * Design, develop, and maintain reliable, simple, and efficient Python services * Architect and implement robust backend systems that handle sensitive healthcare data * Collaborate cross\-functionally to deliver solutions that delight customers * Deeply integrate with clinical trial management and other systems * Write clean, maintainable, and well\-tested code * Drive technical and product decision\-making and help shape our engineering roadmap and architecture decisions Note: This role does not involve training ML models. **Requirements** * You are excited about our mission: getting novel therapies to patients faster * 5\+ years of Python experience you're proud of — across multiple projects, systems, or problem domains. We value growth and range, not the same year repeated five times. * Track record of end\-to\-end ownership: scoping, building, shipping, and maintaining * You are an AI\-native developer — you ship features using Claude Code, Codex, or similar tools daily, not occasionally * Comfortable navigating ambiguity and taking initiative. You figure out what needs to be done – not expect to be told what to do * Self\-directed and low\-maintenance thanks to your strong problem\-solving skills and attention to detail * Background as a software engineer first — we're looking for candidates who have focused their careers on building and shipping production software, rather than transitioning from data science, research, or other adjacent fields * BS in Computer Science or equivalent **Nice to have** * AWS infrastructure experience * Experience with large language models in production, where you personally wrote prompts and used GenAI tooling, such as prompting or guardrail libraries
